  • Trade name of the business;
  • Mailing address;
  • Street address (if different from mailing address);
  • The principal activity of your business;
  • Executor, administrator, trustee, “care of” name;
  • City, state, and ZIP code;
  • County and state where the principal business is located;
  • Name of the responsible party;
  • Reason for applying;
  • The highest number of employees expected in the next 12 months;
  • Closing month of the accounting year, etc.
